APP Palliative Care
POSITION SUMMARY:
* This Advanced Practice Nurse works with a team of Palliative Care Providers and in collaboration with the Medical Director for Palliative Care, to develop and maintain standards of care for patients referred to Palliative Care services. The Palliative Care APN serves as a clinical resource and expert in the area of palliative care and responds to palliative care consultative requests from the Palliative Care Interdisciplinary Team, Home Care or from healthcare providers for patients in need. The APN provides recommendations on complex medical management, medical decision-making and symptom management for patients on the palliative care service. He/ she acts to empower patients and families and works as a liaison to other health care professionals on specific patient issues related to palliative care and their illness(s). The Palliative Care APN is a member of the Interdisciplinary Palliative Care Team and will work with other Palliative Care Team members across the health system to coordinate multiΓÇôInterdisciplinary Palliative Care Committee activities. * The APN will provide specialized assessment, diagnose and implement high quality palliative plan of care utilizing evidence-based treatment protocols to provide patient centered goals of care, symptom management, disease trajectory, prognostic education and counseling. He /she closely communicates with patients referring physician to make recommendations, discuss the ongoing plan of care and prescribe as appropriate. The APN is available to triage referrals to the palliative care service to determine appropriate level of intervention and urgency of intervention and maintains contact with patients providers to assist in care management as necessary to facilitate optimal care. Serve on call and/or weekend rotation as required.
EDUCATION:
Current Registered Professional Nurse and Certification as an Advanced Practice Nurse in New Jersey. Masters degree in nursing required, Doctorate (DNP, PhD) preferred CREDENTIALING REQUIREMENTS: Credentialed as an Advanced Practice Nurse in the State of New Jersey with primary focus of education and training as (check all that apply): * Nurse Practitioner * Acute Care * Adult/Gerontology, or Family * Palliative Care The position requires a collaborative agreement with a physician. The position is contingent upon appointment to the Medical Staff by the Board of Trustees: * Prescriptive privileges for medications, treatments and/or DME * History and Physical Examination or episodic care of patients
EXPERIENCE:
with a minimum of 3-5 years of clinical nursing practice experience, primarily in area of specialty.
SPECIAL SKILLS:
* BLS required * ACLS required, Advanced Certified Hospice and Palliative Care Nurse (ACHPN) national certification, preferred. Expected to sit for exam once meet requirements of national certification. * Serve on call and/or weekend rotation as required
