Position Summary:
To coordinate care for Older and/or Disabled Alaskans for those who qualify under the State of Alaska, Division of Senior and Disability Services requirements, MRDD, CCMC, APD, and Elder Choice Home and Community Based Medicaid Waivers. Responsible for completion of screenings, annual Habilitation and Plans of Care updates; preparation of timely and accurate coding and billing for Care Coordination, while maintaining ethical billing practices and procedures; the development of comprehensive Plans of Care/Habilitation Plans, working closely with finance on initial budgets/ cost sheets information and possible supervision of direct care providers.

Position Qualifications:

  • Associate degree in Social Work, Human Services, Psychology, Sociology, Special Education, Nursing, or a closely related human services field preferred. High school education required. Two years of related experience and/or training preferred, but one year required working with the disabled populations and/or special needs population required.
  • One years full time paid direct work experience within a human services capacity. An understanding of cross-cultural environments/ differences. Act as a strong and positive role model with the willingness to travel throughout western Alaska locations. Prefer Yup’ik/ English speaking but not required.
  • Care Coordinator will have the ability to qualify and receive care coordination number from DSDS, State of Alaska. Professional providers will be expected to provide copies of licenses and certification to the Human Resources Department after one year. Tuberculin clearance at time of employment and annually thereafter. CPR, First Aide required or willing to complete the class.
  • Office Equipment i.e. Computer, Fax Machine, Copier, etc.
  • Case Management and/or previous work in native programs preferred. Care Coordinator needs to be experienced with Division of Senior and Disability Services. Ensure that each assigned consumer has adequately trained providers to provide appropriate support services. Calendar/ scheduling of appointments, meetings, and training for direct care providers in the villages when necessary. Significant experience in program developments. A strong computer skills background required. Experience writing summaries, and synopsis report writing. Ability to operate a photocopy machine, fax machine, shredder, and heat laminator. Ability to fulfill and proofread documents for accuracy.
