The state supported living centers serve people with intellectual and developmental disabilities who are medically fragile or who have behavioral problems. Clerk I performs routine medical records clerical work, in order to support the Individuals’ Records functions. Work involves securing legal documents and Medical Records; handling and transporting medical records; filing, sorting and retrieving legal reports; and protecting the confidentiality of Individuals’ records. Ensures implementation of and promotes compliance with the Recordkeeping Policy. Ensures all records are organized and provides readily access to information relevant to Individuals’ status and supports/services, in compliance with the IRIS Crosswalk. Clerk I works under moderate supervision of the Individual Records Manager with limited latitude for the use of initiative and independent judgement.


Essential Job Functions:
Attends work on a regular basis and may be required to work a specific shift schedule or, at times, even a rotating schedule, extended shift and/or overtime in accordance with agency leave policy and performs other duties as assigned.

Clerk I sorts, alphabetizes and files legal documents into the centralized Medical Record, to provide all available information for review, within two (2) working days. Purges documents from medical records, according to the department Active Record Order Guidelines (AROG) and retention schedule. Performs qualitative and quantitative record audits to identify timeliness and quality of documentation, using both the analysis tool provided by Unified Records Department, the facility, and IRIS (Integrated Resident Information System).

Corresponds with multiple departments regarding documentation pertaining to Records and MATS Audit. Prepares inactive documents and inactive medical records for off-facility storage, by following the HHSC State Supported Living Center storage guidelines and procedures. Initiates and assembles medical records for new admissions, returns from community placement and transfers with reassignments to provide a complete history of services. Reviews medical records content to identify any misfiles in Active Record and that the appropriate ISP (Individual Support Plan) to ISP (Individual Support Plan) purging has been performed according to AROG. Mends and repairs pages in the medical record to protect the preservation of legal documents. Prepares, assembles and prints from facility EHR, Mortality Review Files in chronological order within (3) working days of expiration for committee review, using the Mortality Review Files templates. Provide clerical support to main office in absence of the Supervisor to ensure daily operations of the Medical Records Department.

Other duties as assigned include but are not limited to actively participating and/or serving in a supporting role to meet the agency’s obligations for disaster response and/or recovery or Continuity of Operations (COOP) activation. Such participation may require an alternate shift pattern assignment and/or location.

Ability to use office machines.

Skills in computer applications (Word, Excel & Outlook).

Ability with quality checking documents & detailed oriented.

Ability to communicate verbally and in writing.

Ability to understand and follow verbal and written instructions.

Initial Selection Criteria:
Any combination of education and experience equivalent to high school graduation or GED.

General records management or medical records experience.

Experience with use of medical terminology.


Additional Information:
Applicants must pass a fingerprint criminal background check, pre-employment drug screen, and registry checks including the Client Abuse/Neglect Reporting System (CANRS), Nurse Aide, Medication Aide and Employee Misconduct and HHS OIG List of Excluded Individuals/Entities. Males between the ages of 18 – 25 must be registered with the Selective Service.

All State Supported Living Center employees are subject to Random drug testing.

Flexibility in work hours may be required for this position. The position may be required to work overtime and/or extended hours. Must be willing to work weekends and holidays.
