POSITION SUMMARY:
Responsible for oversight of the clinical nursing, therapy, resource manager and home care aids, ensuring appropriate staffing levels and patient scheduling. Provides direction for compliance with best practice care planning and delivery, with fiscal accountability and responsible patient outcome management. Excels in maintaining and building strong relationships with referral agencies, provider offices, and community partners. Assists the Administrator in operation of the department as delegated; promotes cost-effective delivery of patient care services within regulatory compliance. Rotates Administrative call as needed.
ESSENTIAL JOB FUNCTIONS:
- Collaborates with the leadership team to maintain the standard of care and performance for Home Health services through a primary clinical focus.
- Hires, coaches, recognizes, counsels and terminates staff performance to meet department expectations and develop staff to work to the top level of skill.
- Schedules clinical staff for works shifts and processes time offs as per department policy.
- Monitors and approves timecards for direct reports and acts as a backup to Director.
- Develops and maintains strong relationships with finesse with other departments, entities and referral bodies.
- Facilitates skill development in clinical judgement and interdisciplinary collaboration; ie care conference or observing visits in the field
- Provides skilled clinical interventions under the regulatory requirements of skilled home health care, including effective and complete documentation and communications of associated tasks.
- Positively collaborates with office team to ensure clinical staff are scheduled appropriately per department guidelines.
- Facilitates visit management that balances clinical needs/outcomes and financial responsibility.
- Identifies and prioritizes needed projects and facilitates completion with departmental priorities and financial state in mind.
- Creates and presents annual performance reviews and bi-annual pulse checks that manage staff up.
- Assists Director with finances; for example, creating annual budgets, variance and statistical reporting.

Job Requirements
QUALIFICATIONS:
REQUIRED:
Minimum 5 years applicable clinical experience. Effective communication skills and ability to finesse relationships with community and healthcare partners. Strong problem solver and ability to coach and manage up others. Must be able to prioritize, be flexible and manage difficult conflict situations; Valid driver’s license and private vehicle or proof of reliable transportation to client's homes with good driving record; Proof of current auto liability insurance for self or driver.
PREFERRED:
Valid licensure in therapy or nursing; leadership skills with supervisory experience; and experience in department organization, policy and procedure and staff development.
TYPICAL MENTAL/PHYSICAL DEMANDS:
Medium to high physical and mental capabilities required. Flexibility. Cognitive skills necessary to perform essential job functions in ever-changing, often stressful environment. Must be able to work independently with patients and other professional staff. Must be able to physically maneuver through unpredictable and unalterable physical spaces and to be able to maneuver two flights of stairs when present in patient’s home. Must be able to lift up to 50 lbs.
PAY RATE: $46.14 - $67.35 This is any exempt position and salary is based on experience.
