Job Summary
o Assist in the development of strategic goals and objectives for SAS
o Coordinate and collaborate with each facilities’ pharmacist lead for Antimicrobial Stewardship to ensure seamless and timely implementation of all approved initiatives
o Coordinate staffing schedules for ID clinical pharmacy specialists across the UofL Health System
o Serve as the primary resource to achieve Antimicrobial Stewardship Program goals in collaboration with other stakeholders
o Coordinate review of applicable order-sets, formulary management, System policies, educational statements, etc. that include antimicrobial medications to ensure adherence to best available evidence and internal guidelines, policies, and procedures
o In collaboration with SAS, Information Systems, Medication Safety team, Medication Management Policy Subcommittee, System Pharmacy & Therapeutics Committee, develop and implement antimicrobial clinical decision support to maximize anti-infective medication use processes and outcomes
o Lead the ongoing review of antimicrobial financial performance including the identification and implementation of drug target initiatives as part of the Department of Pharmacy’s annual budget planning process
o Coordinate the development, implementation, and monitoring of pharmacy practice standards that optimize antimicrobial medication use across the continuum of care
o Participate in regular activities of the Antimicrobial Stewardship Program as well as pharmacy participation with the ID Consult Service(s).
o Serve as a resource to collaborate with other stakeholders in efforts to optimize antimicrobial care
o Lead the ongoing maintenance and quality assurance activities of relevant pharmacist delegation protocols (e.g., immunization, extended-infusion of beta-lactams, etc.) as assigned. Coordinate prospective and retrospective audits and provider feedback for antimicrobial orders (e.g., de-escalation, drug-bug mismatches, dose optimization, parenteral to oral conversion), as appropriate
o Participates in efforts related to antimicrobial budget forecasting and shortages and targets related to antimicrobial use
o Coordinate the development of tools to monitor resistance patterns of antimicrobials (e.g., unit specific and service specific antibiograms)
o Lead initiatives to ensure quality metrics are evaluated and monitored to meet Joint commission Standards and Centers for Diseases and Prevention (CDC) recommendations and optimize patient care

Job Summary
The Clinical Pharmacy Coordinator, Antimicrobial Stewardship is responsible for the coordination of antimicrobial stewardship services for the UofL Health System that support safe, efficient, effective, equitable, timely and patient centered use of antimicrobial medications. Additionally, the incumbent will serve as a resource to provide/support leadership and management of antimicrobial and infectious diseases (ID) policies, procedures, and pharmacist practice expectations; develop and implement quality and performance improvement initiatives; serve as pharmacotherapy clinical expert/resource; provide mentorship, education, and training; and contribute to scholarship and research of antimicrobial use. This position has independent responsibility for organizing and planning his or her work to accomplish the daily responsibilities as well as broad goals and objectives of the position. UofL Health administration and leadership understand that support is critical to the antimicrobial success and formally state that they are committed to dedicating individuals, finances, and information technology resources to improve and monitor antimicrobial use. The incumbent will also maintain a practice as a clinical pharmacy specialist in UofL Health Pharmacy Department. The anticipated dedication to administration time and efforts compared to clinical time and efforts is up to 50% and may vary depending on requirements, initiatives, etc. Both work functions are important and dedicated time is imperative to the success of this position.
Responsibilities:

o Collaborate with infection control on drug-related initiatives related to healthcare-acquired infections (e.g., Clostridioides difficile associated diarrhea, catheter associated infections)
o Regularly review and disseminate (when appropriate) literature, list-serves, professional organization communications related to the prevention and treatment of ID
o Serve as a Department of Pharmacy representative (or assign a designee) for interdisciplinary committees, task forces, workgroups, etc., related to antimicrobial use
o Coordinates rounding or consultation services with ID providers and/or pharmacists as needed
o Coordinate and precept pharmacy residents on the ID Consult Service
o Interact with the ID fellows and provide education as needed on the ID Consult Service
o Provide new pharmacist training and orientation related to ID pharmacotherapy
o Develop initial and ongoing antimicrobial pharmacotherapy pharmacist competency assessments and annual review
o Coordinate the development of clinician focused antimicrobial education and competencies

o Serve as a mentor for pharmacists and pharmacy trainees in their professional growth and development in scholarship and research
o When appropriate, under the direction of Antimicrobial Stewardship Program leadership submit applications for research grants

- Clinical Pharmacist Staffing
Demonstrate effective communication and collaborative skills to support continuous improvement
- Work collaboratively with all relevant department leaders and serve as a resource
- Actively participate in a multidisciplinary approach to continuously improve medication handling
- Achieve and maintain satisfactory working relationships with pharmacy leadership, ID Division leadership and staff, nursing leadership, medical staff, Microbiology leadership, and other departments throughout the system.

- Maintain current knowledge or collaborate with experts in these areas for accreditation standards, CMS, FDA regulations, and ISMP and ASHP guidelines pertaining to antimicrobial management.

Education:
- Doctor of Pharmacy degree (required)
- Graduation from an accredited/approved school of pharmacy (required)
Experience:
- PGY1 Pharmacy Residency (required)
- PGY2 Pharmacy ID Residency (required)
Licensure:
- Licensed or license eligible as a registered pharmacist in the Commonwealth of Kentucky (required)
Certification:
- Current certification in Advanced Cardiac Life Support or must obtain within 90 days from date of hire (required)
- BCPS certification or must obtain within two (2) years from date of hire. BCIDP preferred.
