EIS HIV / PrEP Navigator
Position Summary:
The EIS HIV / PrEP Navigator will serve as a team member providing education of and opt-out HIV screening within our community, including in the local emergency department and/or urgent care facilities, as well as the promotion of PrEP uptake and referral to Little River Medical Center and link HIV positive individuals who are not receiving care to LRMC's Ryan White Early Intervention Services (EIS).

Essential Duties and Responsibilities:
Listed are the essential duties and responsibilities. Other duties may be assigned.
  • Responsible for conducting education and opt-out HIV screening using 4th generation HIV ab/ag point of care test in the McCleod Seacoast Hospital ED during the triage process for individuals between ages 16-65.
  • Provide risk assessment, risk reductions education, prevention counseling and linkage to a PrEP provider at LRMC.
  • Provide education and serve as a resource for HIV counseling to person who are newly diagnosed and linkage to a HIV provider at LRMC.
  • Link persons known to be HIV positive to LRMC's Ryan White Early Intervention Services, primary care, mental health, nutrition, and other services that are available.
  • Provide integrated healthcare services by sharing patient information in accordance with HIPPA and Confidentiality laws within and outside LRMC to support an integrated and coordinated patient treatment plan.
  • Collect information and complete all necessary and required documents.
  • Submit reports regularly regarding testing, education, and outreach activities to the EIS Coordinator and PI.
  • Works autonomously with the local hospital EDs, urgent care facilities and community organization staff, as well as the LRMC EIS case mangers, nurses, and coordinator to ensure access to care.
  • Ensure care coordination and transitions with patients/families/caregivers by tracking diagnostic tests, referrals and care transitions to achieve high quality care coordination, lower healthcare costs, improve patient safety, and ensure effective communication with specialists and other providers.
  • Participate in regular staff/team meetings, trainings, and other offsite meetings as requested, and proactively supports PCMH initiatives related to coordination
  • Proactive member of care teams in team-based care initiatives

  • Be comfortable and non- judgmental when working with patients that are homeless, use injection drugs and LGBTQIA+.
  • Strong work ethic and reliable.
EDUCATION and/or EXPERIENCE
  • High School Graduate or Equivalent is required.
  • Experience with HIV/AIDS patients, LGBTQIA+ patients and community resource knowledge is a plus.
