JOB DESCRIPTION - Medical Social Worker - Home Health and Hospice

POSITION QUALIFICATIONS:

1. Certification or licensure in the state where services will be provided and conducted. LCSW

2. Masters Degree in Social Work from an accredited school of social work IS REQUIRED.

SUPERVISORY REPORTING RESPONSIBILITY:

Reports to the Director of Clinical Services

PERFORMANCE RESPONSIBILITIES AND STANDARDS

1. Provides social work services to meet the needs of the patient and family.

2. Complies with legal and ethical obligations for the patient’s safety and well-being.

3. Ensures documentation of communication skills and activities with the patient such as: assessments of emotional factors, counseling for long-range planning and decision making, community resource planning, short term therapy, etc.

4. Provides education in-services in-house and within the community.

5. Participates in departmental education and interdisciplinary meetings.

6. Attends IDT meetings, assists team members in understanding significant social and emotional factors related to the care of the terminally ill patient and family.

7. Assists in discharge planning from hospice service when care is no longer appropriate.

8. Assists in providing bereavement follow-up services.

9. Participates in hospice orientation to employees and volunteers as appropriate.

10. Accepts other assignments as appropriate.

11. Identify evidence based practice on continuous quality improvement

12. Demonstrates an understanding of the corporate compliance goals.
