The Social Determinants of Health (SDOH) & Housing Specialist will be responsible for supporting members in addressing unmet basic needs. It also includes partnering with the Housing Program Manager and clinical case management teams to support housing initiatives by serving as voice of the member and subject matter expert. There is a strong focus on addressing racial disparities, delivering person-centered care, collaboration with the county and community organizations and creating innovation and value within our programs.

Primary Responsibilities:
- Serve as the Housing SME and the Health Plan subject matter expert for housing and sdoh resources, knowledge, and support
- Provide Bi-weekly to weekly updates on any new housing openings private or public, changes in housing funds, public housing authority rather pertaining to local or statewide
- Maintaining a list of housing/sdoh programs and their availability for referrals
- Build relationships and stay connected with Private landlords especially those who are felony friendly (familiar with using platforms like Housing Link
- Ability to be creative and use all resources, programs, linkages to create a housing plan that is person centered
- Serve as a support for the clinical team by guiding them through housing applications, signing up for county assistance, public housing waitlist, and coordinated entry
- Attend all rounds and case consultants where housing and SDOH support can be contributed
- Stay aware of housing programs that have wrap around services like mental health, substance, domestic violence, employment, food, and case management
- Work closely with the Housing Program Manager by communicating any housing and sdoh program needs that are identified and working on ways to better serve our members who are experiencing SDOH needs
- Attend community and UHC events around housing and sdoh matters
- Be present as needed at UHC hubs like B&GC and Sabathia to help support case managers, CHW’s, Housing Program Manager and Growth Outreach Team
- Represent UHC at any all community and CBO events
- Works on special projects related to social determinants of health with Program Housing Manager
- Conducts outreach to and engages members in case management programs. Works with member through the program duration and assist with non-clinical care plan goals
- Serves as a member advocate by identifying underlying needs and facilitates referrals to external resources and services in the health care system, as appropriate
- Engages with clinical team to address member questions or concerns and transfer/refer members to a clinical resource when appropriate
- Independently assess member’s needs, gather accurate and comprehensive member information, and research resources and assistance programs that may be available to members in multiple lines of business. Match these resources as appropriate to support needs of members
- Identify resource gaps internally or for the member, health plan or provider coverage needs, and raise gaps to leadership as appropriate
- Display compassion, respect, and openness to new ideas and opinions when conversing and working with members from underrepresented communities

Required Qualifications:
- 2+ years of experience working with people in the community to obtain housing, education, employment, food, and/or social services
- 1+ years of experience in a healthcare setting or health insurance setting or state/county setting
- Knowledge of federal, state, and local laws and policies that govern housing and homeless programs
Preferred Qualifications:
- Undergraduate degree in healthcare, public health, social work, or similar field or equivalent experience in health care or social work
- Medicaid Managed Care experience
- Project management experience
- Bilingual (English/Spanish)
