Roseville Care Center (Plum Healthcare), is a 210-bed skilled nursing and post-acute care facility. Our wonderful staff provide a range of services that include wound care, short-term rehabilitation, and daily living care for long-term patients.

We have an opening for the Lead MDS Nurse role, hours may vary (typical working hours will be 8-5 Monday – Friday)

Job Description: The MDS Nurse reports to the Facility Administrator and helps to supervise MDS department activities. MDS department responsibilities include management of the Minimum Data Set (MDS) and resident Care Plans. The MDS Director has primary accountability to ensure the MDS Assessments and Care Plans are completed accurately, timely, and in accordance with Federal and State requirements and established deadlines. The MDS Nurse role is expected to work closely with the Director of Nursing and other clinical leadership.

Qualifications:

• Current California Nursing license
• Ability to deal tactfully with personnel, residents, families, government agencies, and the general public.
• Possess leadership qualities, communication skills, and a desire to learn
• Previous experience in MDS required - 1 year

Essential Job Functions:

• Direct the activities of the MDS office
• Possess basic knowledge of computers and associated office equipment
• Keys information into computer systems, including EMR
• Develop and maintain business relationship with Discharge Planners (DCP’s), Case Management team members (CMT’s), Hospital Specialists (HS), and Physicians (Dr.’s)
• Knowledge of current Medicare/Medicaid regulations pertaining to admission and discharge requirements of health care communities
• Willingness to attend and participate in in-service educational classes and in facility functions as required
