We are looking for an Outreach Specialist to join our growing Valparaiso team!
What is an Outreach Specialist:
An Outreach Specialist meets our clients at the crossroads of their mental and physical health to fill gaps and improve engagement, follow-up, and access to care. This position functions as a collaborative member of a client's care team to facilitate linkage to appropriate services to meet the physical and mental health needs of the client through care coordination and referral follow-up

Vital Duties:
- Serves as a resource/contact to clients in accessing myriad of services within the agency
- Facilitiates client access to primary and speciality healthcare, providers, and additional resources
- Conducts and documents follow-up and tracking for all clients receiving an external referral
- Works with members of the client's treatment team inside and outside of Porter-Starke
- Collaborates to implement federal grant requirements and policies
- Facilitates identification of high-risk or at-risk consumers that may have unmet physical and/or behavioral health needs
- Oversees and triages live text messaging access for clients via Intrado and delivers response
Qualifications:
- Bachelor's degree in Healthcare Administration/Leadership preferred
- Readily identifies and believes in the importance of whole person healthcare
- A committment to decreased health disparity and social stigma for individuals with mental illness
Certification/Licensure:
- Must possess valid driver's license
- CPR certified (can be provided upon hire)
- CPI certified (can be provided upon hire)
