Patient Care Coordinator (PCC) Job at Oak Health Center

Patient Care Coordinators carry out several aspects of the Oak Health Center administration, including:
- Under the supervision of the Office Manager, performing duties in assisting clients for intake process, for return follow up sessions, for rescheduling appointments
- Answering phone calls, logging every message, relaying messages to providers
- Managing incoming/outgoing faxes
- Greeting arriving clients, alerting providers, taking payments
- Providing courtesy reminder texts/emails/calls
- Providing triaging - provider, patient and pharmacy
- Maintaining complete patient charts
- Communicating with OM any office concerns
- Communicating with providers any clinical concerns
- Maintaining competency with the EMR system and calendar, DOXY/Facetime
- Assisting providers with letters/forms/consents
- Gaining awareness of safety precautions for the client who is DTS/DTO
- Answering enquiries by phone and in person in a helpful, respectful and efficient manner
- Maintaining appointment schedule and manages rescheduling/cancelling
- Maintaining electronic medical record, including the patient chart
- Ensuring or protecting patient confidentiality
Hours and Location:
- Works up to 40 hours per week at Oak Health Center, Fullerton location.
- Is also available by email and phone to PCCs to answer queries during clinic hours
General Skills and Abilities:
- Customer service and interpersonal skills
- Ability to work well in a team environment
- Good written and verbal communication skills
- Proficiency with a computer including using the internet, the electronic medical record, Google, and MS Office software.
- Speed and accuracy even while multitasking
- Average typing speed: >65 wpm
- Good problem solving skills
- Efficiency, organizational skills
- Reliability, adaptability, and flexibility
Job Type: Full-time
Pay: $18.00 - $20.00 per hour

COVID-19 considerations:
To keep our staff as safe as possible we provide patient care remotely. Our PCCs work onsite in their own workspaces with barriers for personal safety and updated cleaning procedures.
