BeLoved Hospice is looking for a Physical Therapist to join our team of hospice professionals!

JOB SUMMARY

The physical therapist will be responsible for providing physical therapy services to terminally ill patient and their families and reporting to and participating in the interdisciplinary team process. The physical therapist will be committed to the philosophy, goals and objectives of the hospice program and will work under the general supervision of the Director of Nursing and the interdisciplinary team.

DUTIES & RESPONSIBILITIES

Follows an individualized Plan of Care established by the interdisciplinary team for each patient.

Immediately reports any changes to the RN Case Manager and interdisciplinary team.

Reports unusual conditions, observations and pertinent information to the nurse.

Participates in interdisciplinary team meetings

Participates in Performance Improvement activities as requested.

Provides intervention and support as needed related to terminal illness and loss within Scope of Practice.

Provides ongoing education to the Interdisciplinary Team about the appropriateness or contraindications for treatment.

Initiates care on referral from the Interdisciplinary Team.

Works with the interdisciplinary team to develop an individualized care plan to establish measurable goals taking the patient’s diagnosis, functional limitations and abilities, and rehabilitation potential into consideration.

Collaborates with the patient and family during the planning process.

Provides physical therapy instructions and education to patients, families and caregivers, as appropriate.

Adapts teaching style to the individual needs of each patient.

Actively promotes and participates in Fall Intervention Programs for patients.

Instructs, supervises and evaluates the certified nurse assistant care, as it relates to physical therapy, every 14 days.

JOB REQUIREMENTS (Education, Experience, Knowledge, Skills & Abilities)

Maintains licensure by the Oregon Board of Physical Therapy.

Possesses a degree from a baccalaureate or master’s program in physical therapy approved by an accredited organization

Preferred two years of experience as a physical therapist with preference to prior experience in home health or hospice.

Current CPR card required.

Must be able to speak, read and write the English language.

6. Must have reliable transportation and maintain auto insurance.

7. Successful completion of background check pursuant to ORS 181A.195.

8. Must be able to handle frequent interruptions, problem solve, and have exceptional follow through skills.

9. Must be able to cope with emotionally charged situations encompassing patients, families, caregivers, and team members.

Pay: $52.00 - $60.00 per hour
