Program Support Assistant - First Hill Care Management
SCHEDULE: Monday-Friday, 8a-4:30p PST. Role is 100% on site.
At Swedish, we are passionate about healthcare for our caregivers. Full-time caregivers (0.75-1.0 FTE) who earn less than $60,000 per year will be offered the Swedish PPO Medical Plan with a $0 premium cost, including dependent coverage.
The case management associate participates on the Case Management Team (CMT), with the guidance of the nurse case manager and social worker, assists in facilitating appropriate patient placement; utilizes hospital and community resources for continuity of patient care; serves as Utilization Management liaison; collaborates with Medical staff, CMT members, Business Office, Medical Records and other hospital staff; performs other duties as assigned.
- Maintains and expands awareness of post acute settings (skilled nursing facilities, home care agencies, infusion, hospice, acute rehabilitation and long term acute care facilities and others) and community services. Assures current knowledge of admission criteria and payer coverage for such settings and services.
- Collaborates with Case Management staff as requested to coordinate and process referrals as well as any additional information needed for transitions of care.

- Independently and proactively initiates necessary follow-up to ensure a thorough and timely completion of work.
- Provides administrative support to Case Management teams as needed to support work flow process.
- Answers multiple and/or designated telephone lines, prioritizes and routes calls appropriately.
- Documents accurately, thoroughly, legibly during the current shift in the appropriate information system.
- Escalates issues to Case Management team or leadership staff in a timely manner.
- Assists with data capture as needed to support metric tracking.
- Supports the Utilization Management process by completing requests for utilization review, authorization of stay concurrent and post billing denials according to department standards.
- Supports the denial management process through administrative responsibilities including data entry and logging.
- Performs other duties as assigned.
Required Qualifications:
- 1 year Job related experience in a healthcare setting.
- Associate's Degree.
Preferred Qualifications:
- Bachelor's Degree
- Prior experience in discharge planning, billing, utilization review, financial counseling or registration.
- Proficiency in medical terminology.
- Upon hire: Nursing Assistant Certification (CNA), Health Unit Coordinator (HUC), Medical Terminology.
