ESSENTIAL FUNCTIONS:
Maintains the general safety, security of consumers in crisis and aids consumers with the implementation of their individual service and crisis plans.
Primary responsibility includes providing 1:1 assistance, to include direct care services and specialized care in daily living, social and recreational activities as determined.
Serves as the Primary Advocate for the individuals assigned.
- Monitors residents’ progress and completes documentation, identifying problem areas and remedial measures
- Serves as the Primary Advocate for the individuals assigned.
- Schedules and attends psychiatric and medical appointments and monitors follow-up.
- Communicates and collaborates with staff, and treatment providers.
- Provides hands-on instruction to residents identified utilizing evidenced based strategies to assist assigned individuals requiring specialized support.
- Observes individuals’ behaviors and changes in environment to implement crisis plans and assist with crisis resolution.
- Assists in meal preparation for the Enhanced Supportive Housing Program.
- Participates in regular staff training to include specialized training to develop basic knowledge and skills to assist residents in crisis resolution.
- Maintains proper condition, cleanliness and safety of the Enhanced Supportive Housing Program, and responds appropriately to emergency situations.
- Serves as a member of the Enhanced Supportive Housing Team.
- Adheres to all policies, laws, regulations and codes of ethics and confidentiality as outlined by federal and state laws, and agency policies and procedures.
QUALIFICATIONS:
- Bachelor’s Degree in Social Work or related field with three years’ experience in human services.
- Must have a valid Pennsylvania Driver’s License and safe driving record.
- Ability to utilize electronic medical records.
- Ability to transport oneself and/or residents in an insured vehicle in the performance of essential functions.
- Must successfully obtain Criminal History Clearance from PA State Police, Child Abuse Clearance from Department of Human Services, and FBI Fingerprint Clearances.
- Must not be excluded from participation in Medicare, Medicaid, or other federal health care program.
- Ability to transport oneself and/or individuals in an insured vehicle, as program needs dictate.
- Ability to safely operate a variety of agency vehicles.
- Ability to move, assist and/or transfer clients weighing up to 150 pounds, may be required.
- Ability to display an understanding and empathetic attitude when interacting with residents and communication with family members.
- Ability to work split shifts and flexibility to move to various center locations, when program needs dictate.
