Medical Management Health and Rehab is currently seeking an experienced Social Services Coordinator for their long-term care skilled nursing facility located in Macon, GA.

This position will include Admissions, Discharge Planning, Management meetings, Marketing within the community.

Is subject to exposure to infectious waste, diseases, conditions, etc., including TB, and the AIDS and Hepatitis B viruses.

QUALIFICATIONS

• Must be a licensed social worker in good standing licensed by the State, or social services designee by the State and function under the guidance of a qualified Consultant.
• Minimal (2) years previous long-term care experience.

ESSENTIAL JOB FUNCTIONS

General Functions

• Work with Administrator, Consultants, facility staff and family members in meeting identified medically-related social needs
• Investigates case situations and presents information to Psychiatrist and Clinical Psychologist and other members of health team, on resident’s family and social background pertinent to diagnosis and treatment (Care Planning)
• Interprets psychiatric treatment to patient’s family and helps to reduce fear and other attitudes obstructing acceptance of psychiatric care and continuation of treatment.
• Serves as link between patient, psychiatric agency, and community
• Works directly in treatment relationship with patients, individually or in groups, in consultation with Psychiatrist
• Refers patient or patient’s family to other community resources
• Assist in transportation needs of the resident
• Maintains facility waiting list
• Helps patients to respond constructively to treatment and assist in adjustment leading to and following discharge
• Administer Advance Directive program
• Participate in Quality Assurance team meetings.
