Licensed Social Workers for our School Based Services who have obtained a Bachelor’s or Master’s degree in Social Work or related field. Must have license to practice in the state of Ohio (LSW, LISW, LISW-S, LPC, LPCC).

Job Summary: Works with individuals, groups and communities to improve mental health and behavior. Provides therapy to clients in and out of the school environment as well as in the home. Creates and updates diagnostic assessments, treatment plans and treatment plan reviews.

Essential duties include but are not limited to:

• Provide therapeutic interventions to the identified client in order to enhance and foster the client’s optimal functioning and interactions in the home and community.
• To continually assess client needs, service availability and appropriateness.
• Work cooperatively with treatment team members and the family in the provision of services to meet the goals developed in the Person Centered Plan.
• Provide appropriate documentation for service delivery including treatment goals, progress notes and billing and submit said documentation as specified by accreditation standards.
• Maintain required billable hours.
• Complete and maintain all continuing education and licensure.
• Maintains quality results by following agency standards.
• Perform other duties, as assigned, to the need of the agency deemed necessary and appropriate.
