Description: We are looking for a full-time, detailed, dependable, and motivated Transitional Living Youth Case Manager who would be working with homeless teenagers (16 to 21 years old) in an 18-month program. We are licensed by the State of Michigan and nationally accredited by Council on Accreditation.

Responsibilities:

  • Create a treatment plan to provide for youth’s needs, safety, and living plan, using person and family-centered planning guidelines and agency protocols to maximize their physical, social, cultural, spiritual, and emotional well-being
  • Conduct intakes, assessments, follow- ups, and create termination plans to report progress, changes and/or deterioration
  • Facilitate safe living arrangements, appropriate to the age and needs of the youth
  • Provide crisis and short-term counseling to young people and their families in multiple settings, including the person’s home
  • Communicate with youth and adults to provide them with needed on-call support, services, life skills training, resources, information, opportunities, and referrals
  • Maintain contact with teenager in their living arrangement, the housing provider, and families, as required by funding sources and/or agency standards
  • Advocate for youth and families in interactions with community agencies, insuring successful delivery of service
  • Maintain on-going liaison with community agencies, organizations, and individuals relevant to the Child and Family Services Program
  • Provide information to the community and targeted groups regarding our services
  • Follow the Department of Health & Human Services Division of Child Welfare Licensing rules, regulations and accreditation standards
  • Assist in monthly, quarterly, and yearly statistical reports
  • Responsible for assisting with recruitment and outreach activities, occasionally on weekends
  • Complete required documentation, reporting, and billing, and needed

Travel required: Specifically, to and from locations referring homeless youth: schools, host homes, and associated locations in Isabella, Clare, and Gladwin Counties

  • Understanding that children are resilient in difficult situations, especially with support
  • Ability to respond appropriately to the cultural and socioeconomic differences present among the population serviced
  • Ability to develop a mutually respectful partnership with the person served, as well as with their families, so they are helped to gain skills and confidence to address any issues and problems they face

Requirements:

  • Experience working with youth and families (preferred)
  • Bachelor's degree in a human service field (social work, psychology, etc.)
  • Must be in good physical and mental health
  • Must pass all training, tests, and exams required by the State, any government agency, funding sources or the employer
  • Must pass criminal background check, Protective Services check, physical, and drug screening
  • Michigan Driver’s License with acceptable driving record
