Description
AntFarm is a non-profit in Sandy, Estacada, and Mollala providing youth and family services. We are seeking fun, dynamic, responsible people who want to make a difference in the lives of youth & families in our rural Clackamas County communities. The Housing Case Managers provide supportive housing case management services to support housing stability for houseless and homeless youth and young adults and those at risk for homelessness who need to or have obtained permanent housing. The Case Manager connects individuals and households to formal support services, like a doctor, therapist, or soc ial work er and to informal supports, like connecting with family, friends, or faith groups, tailoring their supports to each unique situation.
