Preferred Primary Care Physicians is currently recruiting for a full-time Quality Care Advocate. The Quality Care Advocate will work with our Quality department to assist in providing the top notch level of care patients of Preferred Primary Care Physicians have come to expect. This position is a HYBRID remote position to start with some on-site shifts at the CANONSBURG, PA, home office.
General Purpose of the Position: Works collaboratively with the Director of Value Based Care, Quality staff and practice staff to enhance patient care and satisfaction through quality initiatives selected by the corporation and executing the specific tasks required for efficient and effective operation of the corporate quality program.
Essential Duties and Responsibilities:
- Work collaboratively with the Director of Value Based Care, Office Manager, Quality Care Coordinator, providers, office staff and corporate resources to develop a plan of action to assist in closure of gaps, workflow analysis and process improvement to ensure that quality initiatives are achieved.
- Interpret performance data from a clinical perspective in coordination with Quality Care Coordinator and Assistants, providers, Office Manager, and staff to modify action plan as needed.
- Maintain current working knowledge of Quality measures and make recommendations as needed to ensure efforts are effective.
- Meet quarterly with Office Manager, Quality Care Coordinator and Quality Care Assistant to review quality initiative progress and strategize any need for process improvement and to ensure successful outcomes.
- Utilize provided reports on an ongoing basis to promote closure of open clinical quality gaps.
- Seek clarification as needed for current initiatives or new programs from the Director of Vale Based Care. Communicate the clarified information that may affect work flow to all staff and seek assistance from Office Manager in disseminating information as needed.
- Provide educational support to staff as needed in regard to quality initiatives
- Create, utilize and review various reports through Enli to identify patients who are due for Health Maintenance screenings and/or Annual Preventative appointments
- Utilize reporting tools to assist patient population in being compliant in partnering with providers to manage their health and/or disease states
- Outreach to patients regarding appointments or need for testing as required
- Work with providers and office staff in procuring orders or referrals for patients in achieving positive quality goal outcomes. Assist Quality Coordinator as needed.
- Work with and communicate with the Quality Coordinator each month to review and interpret reports and assess progress of monthly goals and patient compliance.
- Work collaboratively with other divisions to exchange ideas so that resources are pooled and duplication of efforts is avoided.
- For those patients who have a visit scheduled in the near future, Quality Advocate the chart will be prepped accordingly for the provider (orders entered) prior to the visit for any outstanding gaps.
- For those patients with continued outstanding gaps or who remain noncompliant, Quality Advocate will review updated vetted gap lists with Quality Care Coordinator for follow-up and order completion
- Quality Advocate will review patient's chart for appropriate consideration of referrals into multidisciplinary programs. Those suggestions will be reviewed by the Provider at the time of the patient visit.
Education: High School Diploma or GED is required.
Experience/On the Job Training: Certified Medical Assistant or Medical Secretary is preferred
Special Skills: Excellent communication, organizational and clinical skills, independent worker, ability to problem solve, able to work well with Physicians, Office and Quality staff to solve and implement changes, utilize data tracking and trending reports to revise goal setting, strategies, etc. Familiarity with Microsoft office programs, Email, Faxing, Centricity and Population Health Management Tools such as Enli.
