Job Title: Family Advocacy Center Case Manager

Reports to: Program Supervisor

Job summary: Conduct intensive case management services to families that have an open Placement or Intact Family case with DCFS, families that have been deflected and referred by DCFS Protection Staff, and community referrals when program capacity exists. These services are to promote permanency by maintaining, strengthening and safeguarding the functioning of families. This position is an hourly non-exempt position.

Minimum requirements

· Bachelor’s degree in Human Services or related field and at least 4 years related experience

· Bilingual preferred/not required

  • Ability to plan, organize, and prioritize
  • Strong verbal and writing skills
  • Basic computer skills
  • Ability to lift 30 pounds
  • Current Illinois driver’s license and valid vehicle insurance

Summary of essential job functions:

  • Conduct assessment and help design an individual service plan with each program participant
  • Link families to community services through referrals within the agency or other community partners to build and effective support system
  • Enable clients to take responsibility for their crisis, recovery, acting as an advocate and facilitator
  • Develop a confidential and trusting partnership with the participants
  • Monitor progress of individual service plan
  • Keep detailed records, prepare and submit monthly reports
  • Keep an open dialogue with the participants DCFS Case Manager and other professionals working with the participant
  • Network with other agencies to stay informed about services and resources
  • Ability to commute and co-locate in offices in McHenry and Winnebago counties.

Pay: From $19.50 per hour
